We will get back to you to discuss the family’s needs and any support we can offer.

We are very sorry  but all our practitioners are currently extremely busy and many are holding waiting lists, so we aim to prioritise those families in need who do not have any additional support, if a family is already receiving support from Social Care we will not be able to accept a referral for them, if you are unsure, please ring to discuss further. Thank you

PLEASE NOTE:

  • We can only take referrals from families who live in Sussex so please do not complete this form if you live outside the area. Currently we do not have practitioners covering Littlehampton, Arundel, Chichester or Lewes.
  • In order to maintain a free service to the families that FSW supports we are introducing a referral fee payable by the agency referring to the service. The fee will be £75 per family and will fall due at the point of referral. This will come into effect from the 1st of January 2026.
